The 麻豆果冻传媒 Athletic Department believes that athletic and extracurricular participation improves our student鈥檚 lives and provides the foundation for academic excellence.
Athletic Clearance Forms
In order for an athlete to participate in athletics the following forms must be completed:

IEP Records Request
Please fill out the following form to request IEP Records for students who have graduated or no longer go to 麻豆果冻传媒 schools. If you are a parent looking to obtain a current copy of your child鈥檚 IEP, please contact your case manager directly at their school.
Please note: for parents or student鈥檚 over 18 years of age, when picking up records we require identification that matches the name on the student records.
We process IEP Records Requests on a first come, first served basis. Turnaround time is five business days.
