About us

The Medical Home Program (MHP) works with approximately 100 pediatric primary care practices in Alameda County to promote developmental screening of young children and provide additional support to practices serving children and youth with special needs.

Our programs

  • Our team supports approximately 100 pediatric primary care practices in Alameda County.
  • We provide training to pediatric providers and site staff on developmental screening tools and techniques for children aged 0-5 and referral pathways training and resource information regarding children and youth with special needs aged 0-22.
  • We provide ongoing technical assistance to pediatricians and their staff via site visits, email, Zoom, and monthly newsletters.
  • Technical assistance includes assistance with referral pathways and identification of new resources for children and families.
  • MHP identifies critical policy changes affecting services for children and provides information to sites to assist their patients and families.
  • Our team participates as part of Help Me Grow in Alameda and San Mateo counties.
  • Our partners include Alameda County Public Health, AbilityPath, and First 5 Alameda and First 5 San Mateo.
