Employee Assistance Program (EAP) provides virtual, telephonic and in-person counseling. Work and life services: financial, legal, identity theft, health modules, and more. These services are available to all employees, including any person living in the same household. We have switched our EAP provider. Effective 7/1/23 MHN no longer services the District’s EAP program. The new provider is now Optum. Please follow these steps to initiate your EAP services.

Workers’ Compensation Reporting and Site Postings
Workers’ Compensation
When an employee is injured on the job, the employee must report the incident immediately. In order to take care of the employee and also comply with the law, it is essential that the district procedures be followed.
Disability Management
All work related injuries must be reported directly to the supervisor, site administrator or designee as soon as possible. All claims are to be reported to 916-643-9299 immediately.

New Web-Based Training
Our new District’s web-based training is supported by the District’s Employee Safety Committee and SIA. This program will provide significant professional and organizational topics in the areas of safety, loss control, and human resources. This program will give the District a complete training solution to assist in maintaining regulatory compliance, promote safe behavior, and reduce losses.
Concussion Head Injury Report Form and Medical Release

Coach or other removes student from activity and completes form and gives to student or parent. Included medical release must be obtained before student can return to play. Optional to use for other serious injuries.

Effective January 1, 2012, a new law ( Education Code Section 49475), requires the student and parent/legal guardian must sign this Concussion Head Injury Information Sheet before a Student may try-out, practice, or compete in any District-sponsored extracurricular athletic program, including interscholastic, intramural, or other sport or recreation programs (including cheer/dance teams and band), but excluding physical education courses for credit.
